Registered Nurse, RN

The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. 

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES


Patient Care 
1.    Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). 
2.    Regularly re-evaluates patient nursing needs. 
3.    Initiates the plan of care and makes necessary revisions as patient status and needs change. 
4.    Uses health assessment data to determine nursing diagnosis. 
5.    Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. 
6.    Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
7.    Counsels the patient and family in meeting nursing and related needs.
8.    Provides health care instructions to the patient as appropriate per assessment and plan of care. 
9.    Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. 
10.    Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. 

Communication 

1.    Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. 
2.    Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required. 

3.    Communicates with community health related persons to coordinate the care plan. 

Additional Duties 
1.    Participates in on-call duties as defined by the on-call policy. 

2.    Ensures that arrangements for equipment and other necessary items and services are available. 

3.    Instructs, supervises, and evaluates home health aide care provided every two (2) weeks. 

POSTION REQUIREMENTS
    Valid driver licenses 
    Auto Insurance 
    Social Security Card.
In lieu of a social security card one or more of the following documents listed below would be acceptable for employment.
 • U.S. Passport or U.S. Passport Card
 • Original or certified copy of birth certificate issued by a State, county, municipal authority, or territory of the United States bearing an official seal.
    Possess and maintains current CPR certification. 
    Physical or Statement of Good Health within the last year. 
    A Negative TB Test (Part 1 and Part 2) within the last year or chest x-ray would be accepted in lieu of a two-step TB test within the past five years.  
    Background Check 
    Drug Screen 

 POSITION QUALIFICATIONS 
    Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting is preferred. 
    Current licensure in State
    Management experience not required. Responsible for supervising home health aides. 
    Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

The Licensed Practical/ Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. 

ESSENTIAL JOB FUNCTIONS/ RESPONSIBITIES 
1.    Providing direct patient care as defined in State's Name Nurse Practice Act. 
2.    Implements plan of care initiated by the registered nurse. 
3.    Provides accurate and timely documentation consistent with the plan of care. 
4.    Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care. 
5.    Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor. 
6.    Uses equipment and supplies effectively and efficiently. 
7.    Participates in personal and professional growth and development. 
8.    Performs other duties as assigned by the register nurse.  


POSITION REQUIREMENTS
    Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various services for a patient as necessary to meet the patient’s personal needs to promote the patient’s comfort, and a safe environment. The home health aide is responsible for observing patients and reporting/documenting this observation and the services provided.

  

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Responsibilities of the home health aide include, but are not limited to, the following: 

1.     Providing personal care including: 
Baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting patients with dressing and undressing, skin care to prevent breakdown, assisting the patient with toileting activities, keeping patient’s living area clean and orderly, as appropriate. 
2.     Planning and preparing nutritious meals. 
3.     Assisting in feeding the patient, if necessary.  
4.    Assisting in ambulation and exercise according to the plan of care. 
5.    Performing range of motion and other simple procedures an an extensional therapy service as ordered (with appropriate completed/ demonstrated skills competency.) 
6.    Assisting patient in the self-administration of medication. 
7.    Doing patient’s laundry, as appropriate. 
8.    Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.). 
9.    Reporting on patient’s condition and significant changes to the assigned nurse. 
10.    Adhering to the Organization’s documentation and care procedures and standards of personal and professional conduct.

POSITION QUALIFICATIONS 
    One-year combined experience or 
    Completed recognized nurse assistant training program.
    At least eighteen (18) years of age and is a high school graduate or equivalent. 
    Ability to read and follow written instructions and document care given. 
    Displays initiative and able to work with minimal direct supervision.  
    Empathy for the needs to the ill, frail, and impaired. 
    Demonstrates tact, patience, and good personal hygiene.
